The Inventory Report
Is the incapacitated person deceased?
If the Answer is No...

Proceed to Part II and complete the form as directed.
If the Answer is Yes...

It typically means that the incapacitated person died within 90 days of the adjudication of incapacity. The fields on the form cannot be edited, signifying that Inventory report should not be completed in this instance. Notify the court office immediately, using the phone number displayed on the form, and provide the date of death. Click the CANCEL button at the bottom of the form to return to your Dashboard.

After the court records the date of death, the final Estate report and, if applicable, the final Person report are made available. You will then be responsible for completing and submitting these reports.
Part II. Assets (Principal)

Asset Type - Choose an option that best describes the item or group of items that you are identifying. Some of the options are general classifications (ex. personal property) so you have choices available for nearly any type of asset.

Asset Description - Enter a brief summary of the asset. This is particularly important if the selected Asset Type is not specific.

Name of Co-Owner(s) - Enter the name of any individual, other than the incapacitated person, that maintains ownership in the asset. If no co-owner exists, leave this blank.

Asset Value - Enter the amount the asset is worth in U.S. dollars.

You must answer Yes or No to specify if there is any property that is co-owned by the guardian and the incapacitated person. If you answer Yes, additional fields display but they are not required to complete the form.

If you answer Yes, you are prompted to enter the date that the property was acquired and the date on which the guardian's name was added. Depending on the unique circumstances of each case, these may or may not be the same date.
Question 3 - Homeowner's Insurance for Real Property (i.e. fixed property, like land and buildings)

If you answer Yes, the incapacitated person has a homeowners insurance policy, the court may request a copy. You have the option to upload an electronic copy of the document to the report. Alternatively, you can send a copy of the policy to the court office through the regular mail.

Homeowners Insurance Carrier - The name of the insurance company that issued the homeowners policy.
Question 4 - Automobile Insurance

If you answer Yes, the incapacitated person has an automobile insurance policy, the court may request a copy. You have the option to upload an electronic copy of the document to the report. Alternatively, you can send a copy of the policy to the court office through the regular mail.

Automobile Insurance Carrier - The name of the automobile company that issued the homeowners policy.

If the incapacitated person (IP) has a safe deposit box, specify if the IP's name is the only one that has access to the box (Yes, in sole name) or if at least one additional person has access (Yes, in joint name(s)).

If the IP has a box, the location you enter should include the name and address of the bank or other institution where it is housed.

For Question 5b, Are there plans to inventory the contents?, a Yes reply can indicate that you have plans to complete the inventory or that you have done so already.
Part IV. Liabilities/Debts

Liabilities/Debts - Summarize the type of debt that is owed by the incapacitated person (ex. mortgage, credit cards, automobile loan, etc).

Lender - Enter the name of the person or organization to whom the debt is owed.

Value - Enter the amount that the IP currently owes on the debt. This may or may not be the same as the amount that was originally borrowed or incurred.
Part V. Guardian Coverage

Surety Bond - A type of fiduciary bond required by some courts as a way to ensure the proper performance of the guardian's duties. Bonds are often required for individuals named as a guardian of the estate and provides a way to protect the incapacitated person's assets against financial wrongdoing.

The answer to this question will default to Yes or No automatically and cannot be changed. This information is recorded and maintained by the court based on the final decree.

If the answer is Yes, a surety bond is required, you must submit a copy of the bond to the court. You have the ability to upload an electronic copy of the document to the report or you can send a copy of the bond to the court office through the regular mail.
Question 2 - Professional Guardianship Agency or Attorney

A professional guardianship agency is an organization that can be appointed as a guardian for an incapacitated person (IP) and employs individuals that perform the functions of a guardian on behalf of the organization.

If you answer Yes to the question about having liability coverage, you must submit a copy of the policy to the court. You have the option to upload an electronic copy of the document to the report or you can send a copy of the policy to the court office through the regular mail.
Part VII. Financial Plan
Question 1 - Estimated Expenses

Annual Estimated Expenses - The total estimated amount needed, in U.S. dollars, to care for the incapacitated person for a year. This amount should be based on the circumstances that exist at the time the Inventory report is being completed.
Question 3 - Application for Government Benefits

Leave this section blank if no applications have been submitted for government benefits on behalf of the incapacitated person.

Leave this section blank if the incapacitated person does not own any real estate.
Question 5 - Power of Attorney

Power of Attorney - The legal authorization for one person to act on behalf of another individual.

Accounting - A log of all income received and expenses paid on behalf of the incapacitated person.

Agent - A person authorized by another to act in his or her place. In this instance, the agent refers to the power of attorney.
Part VIII. Medical Information

Do Not Resuscitate (no code) order - A document, executed by a competent person, directing that if the person's heartbeat and breathing both cease while in a hospital, nursing home, or similar facility, no attempts to restore the heartbeat or breathing should be made.

If this information is unknown, you must still select Yes or No, but you should enter comments at the bottom of the form that more accurately explains the circumstances.
Question 2 - Health Care Directives

Durable Power of Attorney - A type of power of attorney that specifically remains in effect during the grantor's incompetency.

POLST (Physician Order for Life-Sustaining Treatment) - A medical order for the specific treatment an individual wants in an emergency.

If this information is unknown, you must still select Yes or No, but you should enter comments at the bottom of the form that more accurately explains the circumstances.
Question 3 - Wills, Trusts, Burial Accounts

Burial Account - A bank account, or some other holding, in which money has been specifically set aside for burial or prepaid burial arrangements have been made.
Signature and Affirmation

In GTS, electronic signatures are an acceptable alternative to handwritten signatures. The format of an electronic signature includes an "/s/" followed by the guardian's first and last name (ex. /s/ John Smith).

It is possible that a guardianship could have special circumstances that cannot be addressed fully within the context of the report form. Use this field to summarize anything that could not be more fully explained elsewhere on the report.

Use the Upload Documents icon to add documents requested by the court or to provide supporting evidence for an answer/comment you recorded elsewhere on the report.

Clicking the CHOOSE FILE button opens a popup screen that allows you to browse to any location on your computer where you have a document saved. Only one document can be selected at any given time, but you can repeat this process if you need to upload multiple documents.

Only documents saved as a PDF can be uploaded to the GTS.

By clicking this checkbox you are acknowledging the need, per Pa.R.O.C.P. Rule 14.8(b), to serve a notice of filing to each person entitled to receive such notice as identified in the final decree that appointed the guardian(s). This notice must be served within 10 days of submitting the inventory report.

Service of the notice of filing cannot be completed electronically through the GTS and should not be attached as a supplemental document to the inventory report. Service must take place outside of the GTS by other delivery methods (ex. mail, fax, personal delivery, e-mail).

Save, Save and Close, Ready for Submission, Submit, and View Draft options

Save - Saves any changes you made without exiting the report. You should save often. If you leave the report idle for an extended period of time, the report may time out and you will lose your changes. Unlike the Submit option, the GTS does not check if the report is missing any required information during the save process.

Save and Close - Saves any changes you made to the report and returns you to your Dashboard. Unlike the Submit and Ready for Review options, the GTS does not check if the report is missing any required information during the save process. You can return to work on the report by clicking the appropriate Edit Report icon in the Upcoming and Overdue Reports section of your Dashboard.

Ready for Review - Saves the report and sends it to a designated reviewer. This function is used in some guardian organizations to provide oversight before reports are submitted to the court.

Ready for Submission - Saves any changes you made to the report and checks if the report is missing any required information. If all required information has been entered, you are returned to your Dashboard where the Add to Cart icon displays for the report. If required information is missing, you will be prompted to add the missing details before re-attempting.

Submit - Submits the report to the court. If required information is missing, the report will not be submitted and you will be prompted to add the missing details before re-attempting.

VIEW DRAFT - Generates a draft of what the report looks like at the present time. This feature can be used before submitting to ensure the report appears the way it is intended. The draft of the report opens as a PDF in a new window or tab which gives you the option to print. When finished, return to the tab in your browser for the UJS Web Portal.
